About the authors:
E. Magnus Ohman MD, FRCPI, FACC is the Ernest and Hazel Craige Professor of Cardiovascular Medicine and Chief of the Division of Cardiology at the University of North Carolina at Chapel Hill and Director of the UNC Heart Center.
 Gail Cox RN is a cardiology nurse clinician at the University of North Carolina at Chapel Hill, and the Patient Care Coordinator of the UNC Heart Center.
 Stephen Fort MD, BSc, MB, CHB, MRCP, FRCPC is a Staff Cardiologist at Sunnybrook and Women's College Health Sciences Centre in Toronto, and Assistant Professor of Medicine at the University of Toronto. He is an internationally recognized expert in interventional cardiology and cardiac imaging.
 Victoria K. Foulger RN, BPhil is a chartered herbalist and former cardiac research nurse. She has cared for hundreds of patients in coronary care, intensive care units, and cardiac catheterization laboratories.
